Fellowship to general ophtho

Is it possible for a practicing retina specialist/surgeon to switch his own practice into comprehensive ophthalmology if he/she chose to mid-career? If not, what would be the 'restrictions?' Would it mainly be an issue of decreased referral rates from other ophthos?
