Will it be difficult to find a job in an urban academic or semi-academic environment after completing a residency at a medium sized osteopathic community residency program?

even if you have adequate volume and rotations in large urban hospitals as part of your program?
It's hard to rank urban programs higher on my ROL than my top choice(suburban w decent volume and excellent out rotations), but I am somewhat worried about jobs after graduation (although recent grads have been going where they want)

It depends where you want to work. In my residency program in an urban academic center there were 2 DO's out of ~45. 1 was fellowship trained. In my current academic job in an urban center, we have no DO's out of about 50 faculty. Ultimately it will likely depend on your qualifications, academic productivity, and just how tight a market you want to work in.
