I was wondering if anyone had experience with reaching out to local SNFs to see if they have a need for PM&R SNF consulting. I have heard of some people cold calling locals SNFs and I was curious how others have gone about broaching this. Do you suggest a collections model vs daily rate? Can you do this after hours or does this need to be during business hours? How can you advertise your services if they don't currently have a PM&R doc in house?

Would appreciate any insight, I'm in a high COL, high density city and I've got about 10 SNFs within 1 mile of my current office so it seems like a feasible opportunity.
 
It might be a lot of trial and error with networking, knocking on doors and cold calling. SNFs can be pretty variable on how receptive they are to physiatrists. Some SNFs will want referrals, others looking for pain management, others not interested at all. You might need to make your pitch to the medical director vs administrator vs DON. I did have a few SNFs invite me in through word of mouth, but I was too busy with acute inpatient work to round there.

Typically you would establish an independent billing relationship with your patients and bill the patient's insurance directly, with your overhead being your billing company and malpractice, maybe even a virtual scribe. I would also suggest rounding during business hours during the weekday so you can be visible to admin, therapists, and the primary team.

I'd be curious what establishing that billing relationship would look like practically and with paperwork. I wonder if its feasible as a solo pp doc

It’s not hard, your billing company will take care of submitting all the claims and making sure you are credentialed with insurances. You’ll have to get the face sheets from the SNF’s EMR (or your scribe) and input the billing codes and diagnosis for each visit. It usually takes only like 5-10 minutes of your time each day you visit the SNF.

Would this be something that these snfs would be familiar with dealing with other visiting consultants?
