I am currently a PGY-2 in EM at an institution that just really isn't a right fit for me. I was wondering if anybody was aware of any PGY-2 positions that were currently open or were anticipated to be open as PGY-3 spots for the 2010-2011 academic year. I suppose it may be too early too tell, but it's never too early to start looking.

You also need to start having discussions with your PD. In general, no program is going to take someone without having talks with your PD. As uncomfortable as it is, it has to be a transparent process.
Also, you might be able to solve some of the issues by talking to your PD.
