My teacher who is going to write a recommendation letter for my medical school application asked me if I have the form. Is there such a form?

You should talk to someone in your pre-health committee to ask if they have a special form that needs to be filled out. Since he's asking for the form I'm assuming that this form exists.

^yep. But if your school doesn't have this committee, then AMCAS and schools in general do not have this form.

There is an AMCAS form that must be submitted with LORs. It's used so they can match the letter to your application. It doesn't contain any additional evaluation of the applicant.
