I am planning on applying this year (likely July) if MCAT date holds. My research mentor is a physician in pediatrics infectious disease and has written me a strong letter of recommendation in the past, and I am hoping to ask him for a letter of recommendation for medical school. I'm not sure if it would be a bad idea to ask him for a letter of recommendation now (about 3 months in advance) knowing that he is likely incredibly busy right now.

No, ask now. You have to start asking now because it’s going to take time to get on his radar and will need time to get it done. It will likely take many reminders for it to get done.
