No, you can reap benefits as long as both you and the other party benefit together. True altruism is when you gain nothing or even lose something to let another person gain. There is no such thing as a selfless good deed. Everyone's motive at some base level is inherently selfish. That's just how people work. And someone who goes through 4 years of med school and residency with a half a million dollars in debt certainly isn't going to be altruistic then as they are deluding themselves they will be now.
