I need to write up a hospital order as part of my history and physical class. I was wondering if anyone had a template I could use.

Just a single order (vs. a set like admission orders)?

All orders (or batches of orders) should be dated and timed (military style preferred). Then the order and then a signature along with some way to identify who you are which will be (per hospital policy) your printed name and/or an alpha/numeric hospital identification. Medication orders will have the med, dose, route of administration, and schedule. PRN orders should have an indication. Pediatric orders will need a unit/kg (or meter squared) notation as well and thus a weight for the patient will have to be visible on the order sheet. I'll give a pediatric example:

2. Acetiminophen 160mg/5ml suspension give 5ml PO q4hrs prn fever with discomfort (~15mg/kg/dose)
