How difficult is it to transfer into a 3rd year spot?

I am currently a second-year psychiatry resident at a well-respected California program. My wife is applying for the match this year, and if she matches outside of our geographic area, I will be planning to move away with her. This means that I will be looking for a third-year psychiatry residency spot. Does anyone know how difficult it is to find a spot? Is it dependent on spots being open because others have dropped out of the program? I was thinking it would be easier for me to find a spot in an area with multiple programs such as New York or L.A. I think we would rather live in Oregon or Washington, but I wonder if would be hard to get a spot in these places because there are so few programs. Any input would be greatly appreciated.
