I have plenty of job shadowing experience but what does the application want?
Just the hours and where I did it? Or is there a form the dentists need to fill out?

Under "dentistry/shadowing experience" you put the amount of hrs, the supervisor (dentist), your position (observer), type of Dentistry (general, endo...), description of activity, and the period (from what month/year to what month/year)
