I have plenty of job shadowing experience but what does the application want?
Just the hours and where I did it? Or is there a form the dentists need to fill out?

No forms. There's a professional experience section on the application where you fill in the following:

Dentistry / Shadowing Experience

Name Of Supervisor
Position Title
Type Of Dentistry Observed
Brief Description Of Activities
Total Number Of Hours
Position Type
Month Of Start Date
Year Of Start Date
