how do programs construct a rank list?

I was wondering if anyone could comment on how programs actually construct their rank list. i'm assuming that there must be some process where they try to assign a number to each applicant. for example, they could have a scoring system, whereby various criteria receive anywhere from 0 to 10 points (ie: USMLE, which med school, honors in rotations, letters), and then each candidate gets a total score. but after that is done, i'm sure they go through it again, and tweak the list based on both impressions they got from the interview as well as by gut feeling about applicants.

this is all speculation -- does anyone actually know?
 
very good question. I think that there was a program director on this board who has provided insight into these types of questions in the past. maybe he/she could help?
