How soon do we apply to residency?

How soon do we need to apply for residency, Im reading about getting in the Carribean and attending medical school there. Also hoping for residency in the mainland. When is the third and fourth years arranged so that we can study in the mainland and match in specialties other than family practice? At what point should we be able to make that decision and how much is arranged with the Carribean schools and how much is done with the hospitals?

If I remember, the ERAS opens on July 1st. I do remember that it can be officially submitted on Sept 1st.
