How to address title in LoR author?

For example, if the author is professor of surgery and chief of cardiothoracic surgery, what should I list under title?

It really doesn't matter. I'd probably list the chief position. But don't lose sleep over it

The actual LoR will probably have his/her's numerous titles etc. Just put the one that fits in the box 😛
