How to apply NBDE part 1 retake?

I am foregin dentist. I took NBDE part 1 in 2008 but I failed. Now I am going to retake NBDE part 1. Should I re-send ECE report?

I sendt the e-mail to ADA but no reply.

Please, let me konw the way to retake NBDE
