I missed the NRMP registration deadline... Any chance to find an endocrinology fellowship program outside the match? I was told they need fellows?

Hi all, kinda did a stupid thing not registering for the NRMP fellowship match as a PGY3 IM Resideint who had 3 II. I was kinda unsure about what to do.... but by bad luck I finally decided to go ahead and register for the match... Low and Behold I tried to register for the Fellowship match at 9:33 pm, when it closes at 9 pm. Yeah i was an idiot, but I wasn't sure on what to do. Does this mean I have to wait all the way till next year to register for the match..? I mean are there any open Endocrinology positions? Also, was 3 Interview invites even enough to match? My application wasn't the best as a PGY3 IM Resident. Kinda looking for guidance as i'm kinda confused and upset.

I was told by my mentor that endocrinology isn't hard to find a position with... Even after the match there are lots of positions and PDs are looking for people to fill them... Is that true?

Endo has gotten more competitive. The last few years the positions open after the match have been significantly less.
If you only had 3 interviews you likely were also not a very competitive candidate and there's a good chance you won't match.

It's time to sit down and process how to do better next year

Thanks for your answer... I definitely could have done more research.... I had little research. I also decided pretty late I wanted to do Endocrine....

I mean I was told by my advisor that there are lots of open positions in Endocrinology after the match and programs have difficulty finding people to fill them.

A lot? no. Some yes. My year there were 5 - 10 positions open and 85% match rate.
 
How many places did you apply to? Do you even want to be an Endocrinologist?

I don’t mean to sound harsh on that btw, I mean how strongly do you really feel about fellowship?

I applied to 70 places and I really want to do fellowship now. I know its 2 years, but I want to do Endocrinology, just feeling crummy about my indecisiveness and situation now.

your "advisor" is misinformed. please look at the recent NRMP match data for endocrine...its been in the mid 90%s for a few years now...Scrambling into an unfilled endocrine spot is possible, but not probable... while the 20 or so spots every year are there, they typically fill fairly quickly...maybe your advisor was thinking about nephrology...they have had a >50% infill rate for the last few years.

you applied to 70 places and got only 3 interviews? Its not unheard of to match with that, but you have to ask yourself why did you only get 3 IVs...you should be able to get a 10-15% return...meaing you should have expected 7-10 interviews if you applied to that many places,

What are your red flags?

Being out a year or so won't hurt per se...my co fellow was 10 years a PCP before she decided to do fellowship.

You should spend this next year trying to fix what you can in your application. You should go to some regional and national Endocrine meetings to network and ideally should present a poster at them.

and register with the NRMP in a more timely manner next year.

I didn't have much meaningful endocrinology research. I had no Endocrinology presentations/abstracts/case reports.. I did have an Endocrinology research experience though and a Nephrology presentation. I am wondering that is really my 'red flag' and what caused me to get only 3 II?

Most endocrinologists go into endocrinology because they are interested in the subject… it’s not a high money making specialty… so we look for people who share the same level of interest.

Sounds like your application didn’t really show that you have much of an interest.

What were your LoR like? We’re they from endocrinologists that have worked with you?

it hasn't...certainly not Hem/Onc level!
A case report or a poster presentation can be helpful...again need to show that you have some interest.

Applying to 70 programs and only 3 IVs is concerning...it could be as simple as applying to late in the season...most places only have 2-3 spots and interview maybe 20-40 people so if application comes in late, there may not have been any spots available at programs.

And seriously? Aren’t you the king at taking digs at nephrology? I mean you have dedicated a whole thread to it!
It’s ok for me to call out my own specialty. It’s not ok for other subspecialists to take a dig at other peoples specialty.
 
There is no SOAP for the fellowship match. There's the match, and then directly into the scramble.

If you didn't register for the match, you won't get access to the unfilled program list. But your IM PD may be able to get access, and the GME office should be able to (as long as they have any program participating). You could ask them to share the list with you, although technically they should not.

Per the NRMP results last year, there were 18 spots in 14 programs that didn't fill in Endo. Some of those may be research spots that they will not fill at all (rather than scrambling someone in).

Also somewhat telling is that none of the programs you applied to asked you about the situation. Because you were unregistered, they would be unable to rank you. If you had been high on their lists, they may have reached out to see what was up.

Idk I did apply to 70 places and I applied on time, I wasn't late.
I have 3 LORs, including 1 endocrinologist LOR and some research experiences (not a lot).

But endocrine and nephrology actually have a lot in common and there is an overlap.

The kidney is basically my family business and I seriously considered nephrology… my research in residency was in renal endocrinology and my research mentor isa nephrologist… I think I have more respect for the specialty that RP does.

And an advisor that really doesn’t know the specialty, may have confused the 2… dig wasn’t being made… and the unfill rate is just a fact… easy enough to find in the nrmp match data… while endocrine certainly is not a competitive specialty like cards, gi, hem/onc,ccm.. is middle of the road and not at the level of nephrology and ID…not being a competitive specialty gas no reflection on the intelligence or capabilities of the person… frankly the smartest people I know are nephros.

i did not mean that as a personal attack on you. rather I just commented dryly that it seemed that the irony was somewhat lost with the comment to RP.

anyway your initial point is correct endocrine does not have many open spots but nephrology does
