My questions:
  • Do I obtain this letter myself & upload it to the secondary (as one school has implied)?
  • Do I have the school submit the letter to AMCAS, then forward it to each school? (& will this letter count towards my letter limit at the school?)
  • Who actually has to write this letter (registrar, dean, etc.)?
I searched through some older threads but did not find specifics, but if I missed anything let me know.

It depends on the school and follow their instructions. I would not have it uploaded to AMCAS as then it will be read by all evaluators; the medical school may only require that dean see it and clear it. Usually this letter is referred to as a dean's letter. But in your case it may simply be the registrar sending something to the affect that student was in IA for below 3.0 but that was removed when greater than 3.0 was achieved. I would go and solicit the letter and get a copy yourself to start with.
