I had this happen once last year. I don't think it is as rare as you might think.

Oh wow. Did you actually look at their application or was it an automated interview invite (or maybe their GPA/MCAT was high enough that it bumped it up to the front of the line to check it)? Did they have some special extracurricular or really high GPA/MCAT?

Must have been one hell of an app!!

I've had two schools give an II with a turn around time <10 hrs. In both cases the school had just begun releasing IIs. I've spoken with several others where the same thing occurred, so it definitely does happen. As far as any implications this has? Who knows.

Is it just luck that you happened to look at that app or do you scan through everyone's quickly before a in-depth review?

I have enough screeners that all applications are read within a week, often within a day or two.
I am caught up and these two were in my queue.
