Last Name Changes?

Perhaps not the correct forum but especially pertinent to future radiologists?

Going to be married soon and would like to be addressed by maiden name at work but legally add husband's name to my own. But wondering whether reports are signed with the exact name on your license?

If maiden name is Smith and husband's name is Fader.

Scenario 1: Jane Smith (middle) Fader (last)
Can you use ur middle name as ur professional name or will reports have to be signed as Dr. Fader?

Scenario 2: Jane Smith-Fader
Can you sign reports just at Dr. Smith or will they have to be Dr. Smith-Fader?
 
I assume that your name as it appears in PACS is set by IT and that you would have some leeway when it comes to first and middle names. I don't think they'd be too hot letting your last name on reports be different from that on your medical license but who knows....I mean, you are a professional and can physically sign your name however you want. Why shouldn't it be the same in PACS?
 
Your reports are legal documents. It seems to me that you'd need to sign them with your legal name, although I suppose there could be some specific case law on this about which I'm not aware.
 
I guess I'm a little confused. If you want to be addressed by your maiden name professionally, then why change your name at all? At home you can be already be addressed as whatever you want.

Is it just societal and/or family pressure to change your name?
