Are there many programs that combine DR with NR fellowship? I know my home program shaves a year off the combined DR/NR training, but that's it. Do any programs allow one to save more time? Do most programs have the combined option? Do y'all think that the combined approach is worth it, or does the training suffer?

Just trying to save time and ask those in the know before searching through 100 programs.

I asked this question on another site, and some people said that even if you spent the last yr doing "fellow" work, you don't get CAQed. But that would be great if you could, I'd be all over it 🙂

No, you can't get CAQ in neuro after one of these pseudofellowships. But, if you are going into neuro (like me) it is a good way to get some extra time and training in your field. Of course, the end of year 4 is the boards, so it might hurt you in that regard.
 
To get CAQ (certificate of added qualification) in neuroradiology you need to atleast 1 year of fellowship. So it's atleast 6 years total after med school.
