leave of absence vs. "flexible" curriculum

Let's say something comes up during your M1 year, (i.e. family emergency, illness, pregnancy) would it be "wiser" to take advantage of a flexible curriculum (spread the basic sciences across 3 years instead of 2) or to just take a yearlong leave of absence? Would you be seen as taking an "easier" way through med school if you take the expanded curriculum, as opposed to a year of absence, since expanded curricula are often designed for retention purposes? How would this affect residency applications?
