Letter of Reccomendation from a PA?

Hello!

I'm applying in May and working on getting my recommenders picked out. I'm non-trad and will have 2 letter science prof letter writers (cell bio and biochem). I'm more concerned about the other letter writer that doesn't have any specific requirements. My volunteering supervisor that I have worked with for the last few years turned down my request to write a letter because she thinks it doesn't fall within her scope (she is a MSW). That leaves me with the PA that I worked closely with as an ER tech and my manager from my restaurant job. Would either of those be acceptable?

A work supervisor is ok, the PA not so much.
There are only a handful of MD schools that request a "clinical" letter. The PA would be ok for those.

I worked very closely with an RN and she wrote me a letter of rec, it turned out pretty well for me. However I was the only volunteer in the clinic that she ran so it may have been a different situation.
