Most medical schools have someone on the committee who speaks every language in the world. I had a professor who didn't speak english and couldn't write so he called the dean of admissions and whistled his LOR to her over the phone.

ask him to write it in russian, and offer to pay to have it professionally translated.

Well... that's how international students do it, since they're getting LORs from their profs in their native country, who presumably don't speak English. Their application has to be translated by some reputable service (including their transcripts, etc.). So, that's the way to go, I presume.

The best bet would be to have it translated at the embassy for whatever nation the person is from. I have a hard time believing that the person is completely incompetent in writing. They do have to get published and read journals in english.
