Should the LOR be written essentially on an institution letter head.Because i've got some with the letter writer's name only printed on the letter. Any one applied in this way and the letters have been considered? Please do let me know.

I would talk to the letter writer's administrative assistant (if they have one) or possibly ask your med school dean for assistance on how to hint to the letter writer that this is highly recommended.

Granted, if the letter writer is in private practice and has no direct affiliation with your school -- it makes sense that a letter on that practice's letterhead should suffice.

I would be wary of a letter writer who didn't know that a LOR should be on institution letterhead...
 
The institution head is not an absolute rule...but it lends credibility to your LOR. From the side of IMGs, our LORs are scanned by ECFMG. We have been told that if an LOR appears dubious, ECFMG can send a signal or a note to programs saying they doubt the quality of the LOR because it looked tampered or fabricated. Remember to ask letter writers to sign in nonblack ink.

(Still, I had some letter writer who did not have an institutional head and even after several reminders, still signed my letter in black ink. I still got the interviews though...how much that hurt me, I can never say. But I guess the point is, if you have a chance not to make this a problem, then go and get that letterhead)
 
2 of my 3 LoRs weren't on a letter head... it just had my prfessors names...
i still did get many interviews...

i assume if a program doubts your credibilty they will just call your professor..
