New and Expanding Programs?

What is the word among the EM community about new and expanding programs for the 2010-2011 application year? From just doing a search I have seen rumors about Carilion (Roanoke VA) and MMH (Bradenton FL) starting new programs and I don't know about programs expanding.

I have actually looked into this quite a bit. Currently they are applying for ACGME approval and if everything goes perfect their first class will be starting 2011. I'm eager to see if everything gets approved and I plan to contact a few people involved as I start my ERAS this fall.

If all goes well, my program - King's County (in NYC) will be adding 4 spots next year... we just got the funding approved, so it's in the final stages... that means we'll have 17 EM and 4 EM/IM residents per year! Plus 1-2 International residents (we just started a program with Saudi Arabia). That's pretty awesome - full house! 🙂

UT Houston is exploring expanding from 3x12 to 3x18 or 4x12 and expanding resident coverage out to a second hospital for more diverse clinical exposure.

Timetable probably not until 2012 at the earliest.

I remember hearing rumors that Dartmouth was thinking about a program. It was from a Dartmouth attending so it might be reliable. I've also wondered about U of Vermont. Someone told me their faculty were talking about it this past year. I'm not sure about truth vs rumors with these programs though.

Staten Island University Hospital in NYC is a new program that scrambled its residents this past Spring and will enter the match this year.

As concrete as it can get, the Harborview/UW EM residency WILL interview applicants for entering class 2011. RRC visit is later this month and notice of accreditation will be give just a few weeks before the match. Nearly every attending in the Harborview ED is now EM-trained. This has the potential to become one of the top EM residencies in just a few short years.

Thanks for the heads up. Will we be able to apply to UW through ERAS? How are the interdepartmental politics there now? Is UW more EM friendly now? I hope the ED is less surgery-run now than in the past.

That's very hard to believe. I know of another new program that had their site visit a couple of months ago and is not accepting the first class until 2012. Considering the extreme opposition to EM as a specialty at that hospital, I'd also be very nervous about being there in the beginning.

They just changed hospitals July 1 and are now at Hillcrest in Tulsa. They haven't graduated a class yet. They're the only ACGME EM residency in OK, although there are 2 DO programs. If you're MD and want to get back to OK, they're you're only option.

Does anyone know anything about the OU Tulsa ER program? I was just curious what anyone thinks of the training since it is fairly new.

I rotated there last year. I didn't apply in EM, so I can't compare it to other places, and I rotated mainly at SFH (the old main hospital) not Hillcrest. I think it's a decent program. The attendings are nice and knowledge, and the residents seemed pretty happy. Their resident class is pretty strong, too, even though it's a new program. I think they're graduating their first group this year.

One thing I'm wondering, though. With the switch to Hillcrest, are they still rotating a lot at the other hospitals including, SFH. SJH and SFH are the two trauma centers in town, and there's a surgery program at SJH, so SFH is a place you'd need to rotate at to get significant trauma experience. When I was rotating there, they were pretty hushed about the Hillcrest switch, but the people in charge kept on reiterating that residents will rotate at all 3 hospitals. About the switch, the lower level residents seemed pretty positive about Hillcrest, but the upper level ones really preferred SFH I think mainly because they were used to it.

As of yesterday, the program was not yet on ERAS. With applicants beginning their submissions in 3 weeks, I find it hard to believe that they will be accepting applicants this year. Do you mind disclosing where you heard this information? All publically available information that I've been able to find does not mention a 2011 start for the program.

I didn't realize that. You had posted something about Tulsa Regional Medical Center some years ago (in 2 point type, if I recall), and I thought that that was the program.

I don't know the details yet about applying but I've heard that it will NOT be through ERAS because official word on accreditation won't come until just prior to the match. Not sure how that works if we want to apply to the UW residency and participate in the match. Maybe they can participate in the match but the application just doesn't go through ERAS. Some of you probably know more ERAS/NRMP.

So, the UW is much more EM friendly. (I just rotated there!) Everyone I've talked to (I'm being generic on purpose) states that all the other programs are very supportive of the new EM residency. Right now, Harborview is still run as it has been for the last 30 years, but nearly all the attendings are EM-trained. Again, this will be a new program and experience some growing pains, especially as surgery residents have been doing chest tubes, anesthesia airways, ortho splints/fx reductions, radiology FAST, etc. But, as EM residents start, I'm sure EM residents will start participating and hopefully in a few years completely take over these procedures in the ED as multiple classes graduate. Although this could be a concern, I think ED residents will be more than welcomed in the ED because it's understaffed with physicians right now and many specialties are stretched thin and overworked.

The Harborview ED is set-up as 3 areas: trauma (surgical problems), medic (medical problems), and green (less acute problems). So, trauma is run by a trauma doc (surgery PGY-2) and surgery interns (also other interns) with an attending (EM-trained) overseeing everything. Medic is run by Medic-One doc (medicine PGY-2) and medicine interns with an attending (EM-trained). Green is typically an attending with medical students and some mid-levels. EM residents will work in all three areas and transition to takeover these roles. I doubt there will be any significant changes to this structure in the near future.

Like any new program, there are benefits and drawbacks. But, be reassured that there is a lot of support for this residency and don't concern yourself with the past rumors. It'll be a fantastic residency.

My understanding is that if, for some completely insane reason, the HMC-UW residency doesn't receive official accreditation, they won't participate in the match. So, even if you rank it #1, it won't affect your chances of matching at other programs and your #2 ranked school will become your #1 and so on. The preliminary site visit by the RRC was very positive and everyone at the UW is anticipating receiving accreditation. I guess, if I was running the program, why would I waste my time interviewing students if I wasn't nearly 100% confident that my program would receive accreditation? Hope that helps.

It's been happening for years - new programs don't get their accreditation until well after ERAS season, and, as such, programs will interview, but application is paper/nonEras. This is not new.
