When people say New York Presbyterian, do they mean the Columbia one or the Cornell one?
I don't really understand the distinction/similarities between the two. They aren't the same building and are quite far apart geographically.
Is one of them the "real" NYP?

There's 5 different locations throughout New York - and they're all considered the "real" Presbyterian, because they're all connected/affiliated.

Cornell and Columbia are just 2 of the locations.

NYP is the system that encompasses both. originally, new york hospital = cornell's hospital and presbyterian hospital = columbia's hospital. then 10 or 15 years ago they merged so now they're in the same system. what someone means when they say "new york presbyterian" probably depends on the person you're talking to, but in my mind, columbia presbyterian = columbia, and new york hospital = cornell. if someone says NYP i think of the system as a whole.

Yes. People tend to still call them by their "local" name. So usually the full name "New York Presbyterian" refers to the system, and the shortened names referring to the individuals sites: Presbyterian (columbia/wash heights), New York or Special Surgery (Cornell/midtown), NYHQ (former Boothe Memorial in Flushing, Queens which is affiliated with Cornell), etc...
