NIH or Community Based Research Abroad?

Current MS1 at a mid-tier US SOM. Unsure of what specialty I want to do, but want to prepare myself for a competitive residency. If you had the opportunity to do research at the NIH or go to Africa/Haiti to conduct community based research on aids,HIV,HepC etc for the summer... which of the two would you choose to become a stronger applicant for a competitive residency?

Either one is pretty impressive depending on what your role will be. Do whichever you think you will enjoy and get more out of.
