so I get having an official letterhead if you are a professor, but for the doctor I have shadowed, is it ok if it isn't an "official" letterhead? Obviously it will still have the address/contact info, but it's not going to have a fancy design or anything at the top. Is it ok?

Do you really want to use a shadowing letter for an MD program? They are almost entirely useless.
A DO letter for a DO program is another matter...

Doctor should have stationary for the practice. Some schools may balked at not having it. Besides if this was a short term shadowing, these letters have little substational impact to a committee

I'm using it for a school that "recommends" a physician or clinician LOR. I'm not going to send it to all schools.

For schools that say nothing about requiring an official letterhead, will this likely be a problem? I'm only using this letter to satisfy their recommendation for a physician or clinician LOR. And if it has the practice's address/contact information, why is not having an official letterhead a big deal?

These days, you can make letterhead that looks decent with just Microsoft Word. The letters are converted to PDF and uploaded so its not like you even need to buy any fancy paper with watermarks. There are even fill-in the blanks templates that come with your word processing software. Really, this has never been easier. (By "you", I mean the person writing the letter on your behalf.)
