Calling programs and getting the real story on what supplemental items they want and when they want them can be a hassle, especially if you're applying to many programs. And, the stuff listed as supplemental on PASS seems to be inaccurate many times. To that end, I thought it might be beneficial to start a list of items required by programs in addition to the PASS stuff.

Please only post here if you've personally spoken with the program and confirmed what items they need, and when they need them. Don't post second-hand info. This should keep the information true and help us all when it comes to getting our applications in order.

Loma Linda: Will call applicants requesting supplemental items (don't send anything until contacted). Spoke with them on 7/3.

University of Kentucky: Undergraduate transcripts and 2x2 photo. Send anytime, even if PASS application hasn't been sent off. Spoke with them on 7/3.

University of Nebraska Medical Center: Undergraduate transcripts, 2 post-grad application forms available on the UNMC website (http://www.unmc.edu/dentistry/students/college_programs/postgrad/postgrad.htm), $50 application fee, 2x2 photo. Send anytime, even if PASS application hasn't been sent off. Spoke with them on 7/3.
