Is it extremely difficult to get these without coming from an MD program like most people say? Referring more to like one of the orthognathic fellowships as opposed to a big cancer or cleft one.

no although my program which has a fellowship ranks dual degree applicants above all single degree

Most fellowships are not very competitive, because most people just want to start practicing right out of OMFS residency and start doing T&T. Most of the fellowships cover the aspects of OMFS that don't pay very well. You need to start your life at some point.

What do you mean "sTaRt yOuR lYfe" I thought everyone wanted to do a 6-year program and then flex their academic prestige by matching into derm or full body plastics
