Oregon Health Science's secondary questions?

Haven't you got a secondary from them yet? If you're OOS, there's a good chance you've been screened. I think their 2ndary deadline is like Oct. 15th or something. If you're in-state, then you should give them a call. If you aren't applying there this application cycle, then kudos to you, my uber-well-prepared friend.

The secondary due date is dependant on when you recieve it. (This comes from the director of admissions). My OHSU secondary came about 4 weeks after I was verified at AMCAS, so patience is in order. The only screening they do is to see if you have >2.5 GPA annd >25 MCAT. They want those application dollars, so I doubt you've been screened.
 
"What values and personal attributes gained from your life experiences have best prepared you to enter the profession of medicine? " One page limit.

"Using a general format similar to the one below, give us a chronological account of your major activities and employment, including volunteer and summer activities, for the five years immediately preceding your proposed entry to medical school. " Format is Dates/Activity/Location.
