For prelim in Internal Medicine:
How likely is it to get into PGY2 medicine after a prelim? Is it difficult?
what other PGY2 fields can you get into with that--i know you can get neurology or anesthesiology, but what else? and how likely is it to be able to get into those?

Most of the better IM programs don't take a lot of prelim people...usually just take people about to do neuro or derm @their institutions. And they usually have just about as many categoricals as they want/need...but there are definitely cases where people get to stay on. Another resident may leave or switch specialties. In any case the medicine prelim is more useful to have vs. TY or surgery prelim because it can be used as a springboard to more things. There are also PGY2 spots that open up periodically at different programs. One problem you may get into is that you have to have a certain amount of clinic time/hours over your 3 years to finish IM residency...and I don't know what would happen to someone who didn't do any continuity clinic during 1st year (because it was a prelim year) or how that would be made up.

Getting a neurology spot I suspect is easier than anesthesia, just based on the number of open spots I've seen posted over the years. Average anesthesia applicant I think also has better grades and scores than the average neuro applicant, in general.
