For repeated courses should the "repeat" box be checked off for the original class and the retake or just the retake?

Both.

Refer to the AMCAS Q/A thread in the future. I believe this has already been answered.

And as another verification, right from the AMCAS 2016 Instruction Manual, p43:

Repeat (R)
This selection applies to each completed attempt for any course attempted more than once
