1. Yes, you can use a pen if you're taking the exam before July (starting July, 2011 you have to type your responses on the computer - i.e. no more pens from July on)

2. Yes.

3. All those can be considered one test if you write them all on the same line but if you use separate lines, they count as 3 or 4 (depending on the number of lines you used; you only get 5 lines for the diagnostic workup).

4. FA is sufficient (if you've gone thru med school and passed your exams; more so if you've gone thru an accredited medical school in the US or Canada)

can i print or does it have to be handwritten.as well is point form ok or sentences for hpi?

you should look at the usmle website. They have examples of HPI's in both bullet points and full sentence/paragraph form. Also, they have all the rules there.

You can type or handwrite until July. Starting July, everyone must type.

i think what he's asking is maybe not translating well - are you asking about print meaning block print vs. cursive handwriting?

i don't recall seeing anything saying you have to use one style of writing over the other, the only thing that matters is that it must be legible for the person reading it. personally i would just type the note, because it takes whether or not the handwriting is legible out of the equation and i think it's faster to type your note.

im not a fast typer therefore wanted to use pen. and yeh minimd i was referring to whether i can print or handwrite just in case my hanwriting is not legible, therfore i would prefer to print.
