Welcome to Part 6! Everyone did a bang up job by hitting 10k in less than 12 days--11 days 19 hours and 18 minutes to be exact. A new record! 😀

For those not familiar with what we do here, Molly has been kind enough to put together a nice description.

Forget anything you've heard about the girl who died here from arsenic poisoning. That was a long time ago, and she's still alive, anyway... just busy so she doesn't come in as much. Besides, a little arsenic adds a lot of flavor to just about any dish.

Pretty flowers by the entrance, hey? Lilis, they are. Beautiful. And the music is totally hopping... we have our very own DJ. Looks a little like Brad Pitt.

I guess that's pretty much all you need to know. I'm usually in the corner booth over there drinking a nice glass of wine. You're welcome to join me, anytime. We're open 24/7.

Artificial heart approved

Interesting news story. You sure would think that with all the rejection problems, etc. with transplants that artificial hearts would be better. But obviously they're not. Cool technology though.

OK, you guys asked for it!!

You're a pre-med female shadowing Arsenic for "clinical experience" (meaning, he's a hottie). Arsenic has had a long day in clinic and does his 1000th pap smear of the day. He takes the cells from the cervical canal and puts them to transport medium, where they are then sent to beary, the friendly neighborhood pathologist.

Beary, who has a half hour turn around time, immediately gets to work staining and looking at the cells. Since you are an enthusiastic pre-med, you run over to beary's little scope (hurry, before she signs it out!) You see this:

Beary says, whoa, high nuclear/cytoplasmic ratio in these here cells. Look at those pleomorphic, hyperchromatic nuclei! You're like, wtf, damn pathologist can't speak English, and go back to Arsenic. Arsenic decides to do a colposcopy and identify suspicious areas with acetic acid. He shows you this:

What do you think this patient has and what should Arsenic do next?

Cervical dysplasia - he should biopsy it and send it to you. 😉 The acetic acid turns the bad areas white. 😀

I saw a few of these during my shadowing. 😉

the funny thing is that we were in colpo clinic today (no joke). beary you are a stalker, i'm sure of it now. 😱 😉

A primary amelanotic cancer of the cervix or vagina. Treatment: irradiation, chemotherapy. Referral to oncology.

Edit x 2: I said melanoma when I meant cx!

Aww, megboo, you just had me going on the craziest differential of the day. I was like, well, I don't think it's melanoma, but could it be??

You can get melanoma in the vagina or cervix, and they do stain acetowhite. BUT, the cytology is different (I didn't know any of this until just this minute). Melanoma has characteristic huge nucleoli and the cells tend to be more isolated. Here is a picture of melanoma cytology:

yeah - I went back to read my post and I read "amelanotic melanoma". I thought -- ooooh THERE's something you don't see everyday! But really I posted the wrong word.
 
😳 memory like a fricking elephant for trivia.

wish I could remember to pick up the dry cleaning or something useful. actually, I wish I could remember to drop it off. 😳

The patience to plow through that many pages of Randomness awes me more than the memorization.

Yep, oxeye is right on!! Those of you that said cancer could be right, but the cytology is more suggestive of dysplasia (changed cells on the way to becoming cancerous, but not invasive yet). BUT you would need a biopsy to tell for sure, showing the full thickness of the epithelium instead of just random cells like you get on pap.

So, Arsenic did a biopsy today in colposcopy clinic, and sends it to me who's now playing surgical pathologist instead of cytologist. It looks like this:

There are atypical cells through the full thickness of the epithelium, making it high grade dysplasia, but not invading, so it's not cancer yet. But you definitely want to get it all out with clear margins.

They should only be allowed on specific tracks for one, and two they are a beeaatch to insure.
