Once you start residency and match at a particular program, do they see what you ranked them? For example, if I match at my #2 program, will they know that I ranked them #2 and not #1?

Yes all the programs that ranked you will see where you ended up. I don't think they know how you ranked them. If you told a program that you will rank them 1, they will know if you are lying (if they go below you on their rank list).
But that's not the question he asked. He asked if he matched at his #2 choice, would that program (his matched program) know that he didn't rank them #1? The answer to that question, to the best of my knowledge, is no.
If you lied to a program and told them you ranked them #1 and didn't, and you matched elsewhere and they had you ranked high enough in their slots to match, then yeah, you're busted.

I agree. The program that you match at has no real way of finding out where you ranked them unless you tell them afterwards. However, if you are ranked to match at a program and you don't match there, then a program can figure out that they were not ranked #1.
you stole what i was gonna say!! what darkside says is about all i can think - only that a program that has you ranked to match will know you did NOT rank them #1, but no clue where you ranked their program. i heard some programs since post-match surveys to candidates who could have gone there based on that program's rank list.

I was hoping someone could give me some advice on how to handle this situation.
I have received an out-of-match offer from a program, however my first choice by far is an in-match program(that is one of the "top tier" programs I have seen mentioned on this forum. I should also note that I was invited and attended a second interview with this "top tier" program. I am not sure how they have ranked me and wanted to know if would be detrimental or too bold to call the in-match hospital to ask if they ranked me high enough to match.I am from the UK and am intent on doing Pathology in the US. Please advise on how to approach my number one program.

Hey, if you're going to accept the off-match offer, it's not going to hurt you any to go ahead and call up your first choice and ask them how they feel about you. I'm pretty sure you can't ask how exactly they ranked you (or they may be very unwilling to tell you since that's against the Match rules unless they volunteer it), but the PD may be willing to give you just enough info to keep you happy.
Also, from what I understand the off-match offers are usually time restricted- so you need to make up your mind quickly. If you pull the trigger- you need to withdraw from the match.
As far as your second visit is concerned- it could be a good sign. They call you out-of-the-blue and ask if you wanted to come back? Or was it a result of your questions about the program?
Thanks for the reply. They did sort of call out-of-the-blue and and asked if I'd like to come in again to meet more people. They then set up another schedule to meet a different group of attending's from the 1st interview and a one-on-one with the chairman. I thought it went well but you never know. In the off-match offer there was a 'let us know ASAP' attached. If I called my #1 choice and told them abt this offer do you think they might be more inclined to let me know or would it put them off.
