I received a secondary invite from a school about a month ago. Unfortunately, I haven't been able to complete it yet as it is rather in-depth and I have had some significant distracting factors (several visits to the ED, starting new classes, having surgery, etc.etc.)

Would it be proper to send a letter of reconciliation to this school (apologizing for the lateness, perhaps explaining some of the factors behind the delay)? Or should such a thing even be done after a month?

What should I do to explain to this school that I'm not ignoring them?

No need. I sent in my secondary a month after receiving the invite for Loma Linda and they sent me an interview invite a few days ago.
 
My secondaries for Mt Sinai and Northwestern were both after a month for reasons that you described, and they were my first two interviews. Don't worry too much about it.
 
I've never really understood why people ask this question. The damage is already done (you can't go back in time). So your options at this point are 1) still send in the secondary (1-10% chance you get accepted) or 2) don't send in the secondary (0% chance you get accepted).

TL;DR You miss 100% of the shots you never take so just send it in (there's no reason not to)

I don't think op was debating whether to send it in or not. He just wanted to know whether to send an email/letter with his app to explain the delay.

If that's the case then don't send an explanation. It only draws attention to the delay and I doubt most ADCOMs would pay attention to the dates sent and received. More important things for them to look at.
 
Okay, little update that I would like your comments on...

I had to go to the ED again. This time I ended up with a nasal canula because my O2 sats were too low. (oh joy! 🙄) Unfortunately, this excursion and the recovery from it have caused me to miss one of my deadlines.

Now, the big big big BIG problem is that this is my IS school, only 15 miles from my house, and the one I have the best chance of getting into. I'm going to send them a letter begging for leniency, but I don't know exactly what to say.



Any ideas or comments from the good people of this forum?

Try and ask for leniency... you have nothing to lose at this point. But don't expect a positive response.
