Started Med In Germany, Would Like To Finish in the UK or Ireland (UK Citizen)

I wanted to see if anyone had any pointers on how feasible this is. I'm a British citizen, but studying in Germany, and my legal residence is in Germany. Did not grow up in the UK, but English is my first language. Are credits accepted as transfers, or does one have to go via ERASMUS for any chance, or what?

I'm fairly open to anywhere in the UK and Ireland, to be honest.
