Supplemental forms

I was wondering do supplemental forms come with some forms that
the professors are supposed to fill out/check and attach with the
letter of recommendations ?

Ive talked to my LOR professor and one of them likes to know if there
is any forms/ questionnaire they will be filling out. If so he was saying he
could wait and do the questionnaire and LOR together.

Should I get the LORs before I get the supplementals?
Do professors have anything to do with supplementals?

you submit your LORs at the same time as your secondaries.

i haven't had any questionnaires, but if you plan on using a letter service they might need a cover page when the prof sends the LOR to them.

get your LORs now and start an interfolio account. it will save you much time and worry.

How will you be sending the LORs to the schools? Is it through your pre-health committee? Or Interfolio? My committee had a form that they gave me to give to my LOR writers, then the LOR writers turned them in to the committee and the committe put them together and arranged for them to be available via download to the schools (through VirtualEval). For interfolio, I think that once you sign up for the service, they have an LOR request form that you should give to your LOR writer, who transmits it to Interfolio, who then makes it available via download to schools. The only school that I recall applying to that had it's own form was EVMS. But that didn't apply if your letters would come to them via VirtualEvals (no idea if it would be necessary with Interfolio though I doubt it). Regardless, request your letters asap and get them together. This can really hold your app up if an LOR writer takes their time....
