Not from your area so I can't offer any specific school advice, however I am applying as a second career so I'll tell you how I did it. My chem was much older than yours, so I retook both Chem I and II, I think the general consensus is if it's more than 7 years old you should retake it. If you got an A before you should just be able to brush up without retaking.
I just registered at my local university under a 'Non-Degree Graduate' status and started enrolling in classes (your university might call it something different). Each school has different pre-requisite requirements, so research what schools you want to go to and their requirements. You have the basics right but you'll probably need to take more bio (e.g., biochemistry, microbiology, anat/phys).
