UCI letter of recommendation confusion

Hi,
I'm at UCLA and am going to send a packet of letters to UCI. If that's the case, do I still need to fill in the stuff under the letter of recommendation section that asks for...

Submitted By Affiliation Relationship Date Received

or is this only if I am submitting individual letters from the recommenders themselves. Thanks!
 
I hear from the berkeley folks that packet is like individual letters. just put down each writer's name and date recieved(bs it)

Yeah, and just to further verify it, I called UCI yesterday and they said "it behooves you to write the name of each individual writer." So yeah, list 'em all.

On a side note, having this packet of letters has turned out to be a bit annoying, because each school has different requirements. Some schools (like UPenn I think) want you to list a packet of letters as a committee packet, but other schools (like UCI) want you to list each individual letter. Some schools don't really specify, and I'm left to wonder if they'll view me as complete after my packet of letters arrives... guess that means I have to call more schools. 🙁
 
we don't write the date received ourselves right? that's for them to fill in when they receive it?
 
Correct, no dates received i believe.

Are you guys trying ot do this app as quick as possible to beat the wave?

Sometimes i hate SDN, too much pressure for all you elite(l33t) pre-meds. I am so pressured with all of you guys by my side. But again, med school is all about stress... live it, love it

I'm sure if you submit it by the middle of August, you'll beat the wave. Don't worry, just stay on top of things and try to have a 2 week turnaround or so. 👍
