I read an older thread discussing this (2004), but does the SF match still today require undergrad transcripts? How much weight does it actually hold?

As of a couple years ago when I applied, yes they still had a requirement for my undergraduate transcripts.

As far as the weight it carries, I'm sure this varies per program...from not at all, to some weight...I highly doubt this would ever make or break a candidate that was very strong otherwise (or the inverse, help a very weak candidate).

Total crap you have to give these, as far a weight I doubt they carry much unless you bombed something, ie one real ball-buster commented on my C in freshman biology.
