Hello,

Where do you guys get this residency form? Is this a regular affidavit that I can write myself saying the address and the state I live and put a public notary stamp? Do all applicants have to do this or only Florida residents?

Thanks

The residency classification form came as part of the secondary application. I don't know what the rules are for filling it out if you're from out of state, but you can email Mr. Larkin if the instructions are not clear. It didn't require a notary; they ask for your driver's license number, your license plate number, and your voter's registration number as proof of residency.
