Hey there, active duty doc here looking to do some volunteering at my church for a free clinic they provide. Is there any sort of approval you all are aware of? Do I need to go through the whole ODE approval for volunteer work?

Meh. Considering the plethora of shady off-duty employment that we all engage in (and the lack of any real disciplinary measures if/when caught), I don't think anyone is going to bust your chops over volunteer work, especially if it doesn't interfere with your AD day job.

Better question is, who's covering your mal-practice insurance?

Also, I assume you're residency trained and board eligible/certified? Please don't tell us you're a GMO looking to do this.
