holland, it was funny because I went there with two people, and on our way back the girl I was with didn't know we were in the netherlands for the whole time. So we had to explain to her that amsterdam is the city and netherlands or holland are two names for the country.

It was a fun trip though, I went to the infamous red light district and got into a fight with a pimp over there because the girl I was with was taking pictures. I am not a big guy but I think I scared him a little bit.

The red light district sounds fun, I heard the "eye candy" there is not so great.

That's the Hairy Old Man! Better known as Hairy Cell Leukemia variant of CLL...B cells are TRAP+, big spleen and pancytopenia.

I'm doing heme today too 🙁
 
quick question.... it might be kinda dumb....

but I was studying CT and Derm out of Kaplan's Organ System books and they keep referring to muscles as being pre-axial or post-axial..... anyone know what they are referring too? Such as "postaxial muscles in the lower limb are anterior in location; postaxial muscles in the upper limb are posterior in location"

yeah, apparantly people from holland speak dutch and that is different from german. I learned alot over there, like how being liberal is very relative. I mean compared to alot of eastern countries we might seem liberal but in europe americans are very conservative. I think everything is legal over there and their is no censorship at all. what I liked over there too was that everyone seems to hate bush and if you say something like that you hate his guts they will love you right away and invite you to their secret rave parties. Plus the people over there are much better looking, I don't know but it seems like only the not so attractive people immigrated to america, so there is some selection bias in our population favoring the ......., I guess I should stop there, I know its wrong to be superficial, its what is inside that should count

It was a nice trip overall, I wish I could re-live last summer. It was alot better then this one so far

This is only a guess...it may have to do with how the arms and legs rotate during the embryonic period. The arms rotate out and the legs rotate in; in both cases, the extensors would be on the outside (postaxial???) early in embryogenesis and end up in their respective places after rotation.

In other news, did Q-bank literally just now crash on anyone else like it did to me?
 
I only remember that pre-axial is cranial in direction and post-axial is caudal in direction

Actually, if you remember from class (and FA), Staph Aureus is still more common. Salmonella just happens to be found in Sickle Cell-ers. 😀

First Aid says in three different places that the most frequent cause of osteomyelitis in sickle cell patients is Salmonella. pp. 200, 232, and 387 (2005 version). It says to assume Staph in the absence of more information.

Class probably reflects the Detroit IV drug use population where Staph infections run rampant, and some of those people are sickle cell patients.

close, the answer I was looking for was CLL, yes CLL

My brother in law had that question on his internal medicine boards, he is renewing his license which you apparantly have to do every 10 years. So I guess you should know this fact till the day you die, or stop practicing medicine

OK... maybe I read that in the path BRS or Step Up then.
 
the pistons are awsome, I see us winning the eastern conference and then losing the championship. I love D-troit, this is the coolest city in america. Unless the pistons lose the next game, then I am just moving to miami

I Completely agree- this stinks! I have been studying like crazy and it seems like I am better off just guessing on the exam. I will probably manage the same grade.
I think I am going to go cry myself to sleep now. 😴
 
so we all agree that Q-bank is evil. What does it take to get a 70%? I feel like I am going to fail this thing, or do really bad. Hopefully a 65% means average on the USMLEs, if not then I guess I am not average material

God help me

A source close to the investigation says that they'll mail us something in a week or so.

And whenever someone from any administration gives you a timetable for something that will be beneficial for you (as in this case), you need to apply the 2x+2 rule that I came up with years ago. You take the original timetable (1 week), double it, then add two weeks, and that'll be about the time you actually get it.

Anyone who gets this, I will be impressed. I was completely clueless?
 
isn't it the tubulin disease, where all your tubulins are messed up to your organs are all in the wrong place, and you can't get rid of your mucus so you can't breath well

immotile cilia + situs inversus

I got this wrong in a question book - the situs inversus means that the heart is on the right side -- but the vessels are not transposed. In Kartagener's, the aorta still comes off the left ventricle and the pulm. artery still comes off the right.

Good job!
It is a disease where you lack the dynine arms (9-2 microtubule), so you have impaired ciliary action, and therefore you have problems getting rid of mucus.
I don't ever remember that being taught in class-- I am impressed. :laugh:

Yeah, its a dysmotility sydrome. Cilia don't work so you have respiratory problems, infertility because the spermy tails don't work and for some weird ass reason all of your organs are on the wrong side (i think its called dextrocardia or something).

Oooh....oooh. I know this one! It's immotile cilia syndrome. There is infertility due to imotile sperm and lack of movement to guide the egg down the fallopian tubes. Also, increased resp infections due to lack of cilia motion sweeping the debris out of the lungs. I think that also causes the heart to be on the opposite side. Forget what that's called.

I am soon to be MS1. I was wondering, were you guys given a stethoscope your MS1 first week or month at Wayne (by a sales rep or something)? If so what kind?

Littman Cardiology III, Littman Master Cardiology and Harvey Elite seem to be the most popular ones right now. Which one do you guys recommend?

Alas, no one gives you a free $150 stethoscope. There are some giveaways via raffle during your orientation week from AMA and AMSA, so hold out on buying medical supplies. Later in the year, sales reps will be coming to give the pitch for their products and give you very good deals if you order throught them, including free stuff and rebates. So, wait until then.

All you need for Year 1 is a stethoscope, and the vast majority of us have Littman Cardio III models.

I definetly agree. Kaplan charges us so much money. I spent two hours today trying to figure it out. I think they should refund us our money. Those bastards. 😡

Anyways, I watched the national spelling bee. Anyone see this? These kids are like 13 and in 8th grade and spell words I never even heard off. There was one word on there, that we should know, "icterous" (sp?).
Apparently, the longest word in the dictionary is some 45 letter word, and this indian kid could spell it.
These kids should play with leggos and play station - instead they are studying and memorizing the dictionary. I think their parents should be brought up on child abuse charges.

The OSCE and final grades are up for 1st year clin med. Ugh. I'm average.. Again. I hate being average. Why can't I be outstanding? Grr....

Thanks Soliel, I guess for once Wayne did something earlier than I thought.

It's a little depressing though, that was not an impressive OSCE performance for me, damn slow talker SP.

Anyone have any idea when the year 3 calender will be updated? I know it will be only 1 day off from last year's, but for some reason it's really irritating me that they take so long to do EVERYTHING. Just like I emailed Dr. Roe 3 weeks ago and he still hasn't wrote back. 😡 I feel like walking into Scott Hall and punching everyone I see in the face.

You'll be waiting a long time for that reply from Dr. Roe. I emailed him about 2 months ago w/ a 3rd year rotation question and haven't heard a thing. He's seems to be really enthusiastic about his new position as dean.... 👎

Hi- I emailed him last year and I am still waiting for a reply.
I did once catch him in the hall way and he answered all my questions - I guess that is the trick
 
Hey....I asked this question before, but got no response so I'll try again. When does most of the significant alternate list movement occur? They haven't taken anyone off after May-15th and I KNOW the class is not "full" because I personally know about 6 or 7 people who withdrew their acceptances. Any information would be much appreciated.

Anyways, expect to get accepted anytime from now till the week of school orientation. Hopefully you get in, alot of people in our class were in your shoes so don't worry to much

Yeah I know that many people withdrawing...and I don't blame them with acceptances to schools like NYU, Northwestern, Michigan etc.

I don't think Dr. Roe knows how to use email. I have emailed him 3 times over the last year and I never heard a response.

I guess it depends on the year. The past two years, they said they had a very low withdrawal rate and this year it is very high, plus they should be accepting 20+ more students. All you can do, is just keep calling, possibly meet with the Dean of Admissions and assure him you want to go to Wayne State really badly.
You know, they accept people all the way into the first week of class

Same here, but he reads it, after one I tracked him down in person and he had obviously read the email, just apparently decided it wasn't worthy of a response

What is going on here? Is Wayne notorious for telling different people different things? Because when I just called, I was told that only 5 people have been offered off the alternate list and that the class is "full". I find that extremely hard to believe because I personally know more than 5 who withdrew acceptances to Wayne. And the fact that Wayne intially accepts the exact number of the class size before May-15th makes it impossible to only have 5 open spots. In years past, a large percentage of the class has come from the alternate list....I don't see why that would change this year, and I don't know why the admissions office would straight up lie either. Oh well, I guess all I can do is wait.
