What classifiers doctors use for EHR in UK?

Hello everybody,

I would be very grateful to you if you help me out on this question.

I’m a student writing my diploma on software security protection for personal health information in the UK. The aim of my program is to be able to find medicine names and diseases on a computer (for example a list of medicines prescribed to a patient).

I did a research and found that there are several disease classifiers that doctors might use for EHR (electronic health record):
1. SNOMED CT
2. ICD10 (UK version).

These classifiers are different, so I’m confused. Could you please tell me what classifier is really used for EHR?

I also found 2 medicine dictionaries:
1. NHS dictionary of medicines and devices (dm+d)
2. Medicines from NHS posted here.

Could you please tell me what dictionary a doctor uses in EHR to prescribe a medicine to a patient?

Doctors use the NICE BNF (British National Formulary) drug dictionary to prescribe medicines to a patient. Can't help with the rest, sorry.
