They are separate components of your application. Working with patients is essentially mandatory. Doing scientific research is strongly encouraged.

If you are doing clinical research, where you engage current patients personally in the course of a scholarly investigation, it can "count" as health-related experience, just as volunteering or having a job in a medical environment might. The key point in all these potential scenarios is that you are interacting with sick and injured folks.
the point being that they should be patients and you should be physically in their proximity. Research with healthy volunteers would not be "clinical" experience because these folks are not patients in the context of being sick or injured or in a role of seeking health care (e.g. well baby check ups at a doctors office or a healthy woman seeking care in a family planning clinic). Research with patients can be a clinical experience.
Medical missions where you are working with other doctors and health care teams delivering medical care in third world nations also counts, and this looks especially good for schools that focus on under-served populations (LLUSM, UCLA, etc.). Just make sure it is with a reputable organization.
