What counts as a matriculation?

I applied to medical school two years ago, was accepted and granted a deferral. I did not end up taking the deferral. I never submitted an application in AMCAS for the deferral, although the school listed me as pre-approved for a deferral.

If I apply again, will I have to select "yes" to the question of "have you ever matriculated at a medical school?"

you're likely done unless you have a really amazing story of tragic illness or deployment
 
I think matriculation involves an expectation that you are going to show up for the first day of class, but that is a fairly uninformed opinion, and you sound like you are in a situation where you need technical accuracy.

Call AMCAS and ask them to help you determine how to answer that question.
 
I think the more pertinent question is why you didn't matriculate. The formal definition of matriculation is not what the schools will be looking for.

Do keep in mind that there will be evidence of your acceptance in AMCAS.
