What does it mean to accept an offer outside the Match?

What does it mean to be offered a position "outside the match"? I really don't know, seriously. I am not a medical student but a non-MD/DO medical professional hoping to begin medical school in 2014.

How does this usually happen? Is it usually when a student rotates and the PD offers them a position? Are all residency programs allowed to make offers outside the match?

Programs that traditionally did not fill, or had trouble finding good quality applicants, would sometimes offer positions outside the Match to certain applicants. Students currently enrolled in MD schools are not eligible for positions outside the Match; if they end up as interns somewhere, it has to be through the Match.

The only people that are allowed to accept pre-match offers are IMGs/FMGs and DO students applying to an MD residency.

As sylvanthus said, though, the NRMP is making all pre-match offers illegal, so it won't be an issue after this year.

I believe that if a program wants to participate in the match, all positions have to go through the match. Otherwise, programs are free to fill outside of the match.

This. There are programs out there (often in competitive specialties) that can and do take US MD students outside of the match. It isn't the norm, however.
