He’s right. We are mostly accustomed to a hospital based model but some groups have just an outpatient contract or contracts, cover 3-4 rooms per doc, lots of private insurance. High efficiency. Make $$$$

Anyone see Warren's single payer health plan? Abolishes private insurance and pays only current medicare rates with no increase. Seems like it would be highly disruptive to all groups. What would a Mommy track job pay under Warren's plan, 120K?

IF she was elected AND dems win senate AND house, it is possible. ASA would then have to do some real work to fight for a massive adjustment to Medicare rates for anesthesia, otherwise half the specialty would quit.

Mommy tracks would pay next to nothing which will then lead to cries about women making less than men which will then prompt the govt to mandate equal pay for all which then means men will just make less.

the question is what happens to all anesthesia reimbursement. From a physician POV we'd want hospitals to offset that drop in revenue with subsidies. If they didn't and you only got CMS revenue to pay the anesthesiologists I bet you'd be look at <$100K for mommy track.

depends on what cases the mommy did!!
If she did all endo cases she could make 500k depending on the insurance.
If she did cardiac.. then perhaps youre right.

No worry. There are always ways to make money.

Ketamine clinic, hydration van, stem cell clinic.

I was listening to podcast while driving: stem cell clinics are very profitable. A vial of stem cells is sold at 1000-1500$ and the doc charges >5000$ for an injection.

I donated my son's cord blood hoping it may help somebody, especially a kid. Instead, it is mostly likely to be used for profit.

F*ck it.

It won’t happen because first and foremost I don’t ever see Democrats taking the Senate and at the rate they’re going they may lose the House so we may just have our first female President who won’t get anything through Congress.

you can't abolish private insurance, you would just have to make the public insurance so good that very few would want to pay for anything beyond it
 
An electrician came to my house for a minor repair this week. He was in and out in under 40 minutes. I paid him $150 in cash.

Mommy trackers and people who are sick of taking call as an employee are better off putting in a little effort to find independent contractor and per diem type assignments. There is a lot of need for it out there and rates are much higher than what you’d get as a day time employee. Benefits are obviously the wild card, but if you have a spouse who gets benefits then it’s a no-brainer.

Look at Washington. Your city and state governments. They are **** shows.
There is no heirarchy. Massive corruption. and very little results for the amount of taxation they collect.
The federal govt cant even run the VA properly. The VA has a budget of roughly 200 Billion dollars to cover 10 million veterans.
There are huge waitlists, vets are turned down, there is massive corruption and scandals, on and on and on. And, NOBODY can fix it. They cannot do it. There IS NO ANSWER for the fraud that is the VA.

what do you think medicare for all would look like? How much waste and abuse and fraud do you think there would be in that kind of system?

I don't understand why government can't give veterans federal employee insurance and let them go to any hospitals that accept the insurance?

Convert VA to regular hospitals.

Endo will probably go back to nurse sedation nationwide like it is on the west coast.

The answer is the free market. While this will NEVER happen due to politics the government is the problem and not the solution. The govt. should be providing a small "stipend" or "supplement" to those who are older (Medicare) or poor (Medicaid) then allow the free market to work. This means each plan is customized for each family and insurance can be sold across state lines.

Instead, what we will get is a Medicare "buy-in" option on the exchange. Warren will settle for this because she won't be able to pass a mandatory "Medicare for all" option in her first term. She may be able to pass a "buy-in" for those 55 and older to see if this buy-in option is viable for the entire country.

That's an arrangement not unlike an anesthesia eat-what-you-kill setup where you do a nicely paid case and then sit around for 2 hours until the next. That electrician didn't get paid for driving his van to and from your house. How many other appointments did he have that day?

That is absolutely the answer.

Having one hospital system dominate in a market by owning all 4 hospitals and ALL the physician practices does NOTHING to decrease cost. If anything it increases cost.

1000 bed hospitals should NEVER exist.

3 350 bed hospitals instead owned by different entities is better.

strictly enforcing stark and anti kickback laws is the answer.
The Affordable care act that put consolidation on Steroids is not the right move.

Of course. My point is not to say who’s making more. He has a number in his head to both stay competitive and to earn a living. Everyone should know what that number is to them. The point is to value your time well when figuring out working arrangements with employers. I think if you are not going to be a partner in a private practice then you have to start thinking like that electrician and know what your time is worth.

Edit: I do think $110-120 an hour even with benefits is way too low even for a 7-3 type gig. That is less than CRNA pay.

Edit #2: Any private practice that is hiring a significant number of mommy trackers is subsidizing partner income with those mommy trackers. They may say they are doing it because that’s what “the millennials these days” want, but the reality is it makes good business sense for the partners. The majority of well-paying anesthesia happens between the hours of 7-3 anyway. The point of knowing this is someone looking for a mommy track position should know that you do have some negotiating power. It is likely that a group wants an employed 7-3 person more than they lead on. I still recommend going the independent contractor route, but some people like the security of being an employee.$110 as cited somewhere above is way too low.

It’s happened in retail and media over the last few decades and healthcare in the last 2 decades. We could all be working for Amazon Healthcare in 20 years.

In our case, it is absolute government intervention and regulation that lead to consolidation(and collapse of many fine institutions) in our industry. There is no free markets in our healthcare system.


You cant even compare media and retail with healthcare.

Stay tuned though, the chickens are gonna come home to roost.
There are many fine entrepreneurs who are going to disrupt these mega hospital corporations..

There would be a lot of early retirements, myself included, and there would be no mommy track as it wouldn’t pay enough to attract anyone. It will be the 90’s all over again, except the super partners won’t be getting rich on the slave class this time. We will all go down together.

there are around 350k employees in the veterans administration. I bet at least 1/3 or more are non clinical positions. Maybe even more.

On top of that, I would venture to say that 20 % of the VA work force is unemployable outside of the federal govt.
They will go on medicaid and food stamps if the VA shut down.

Just curious how much are part time jobs paying now with the recent increase in anesthesia salaries ?
Women with kids still generally take lower offers like hourly $200/hr-250/hr at some places I know w2

It’s just ridiculous what the settle for.

Actual Part time w2 gigs are getting rarely for women docs. Most have gone hourly.

The only part time w2 gigs women take at state or federal jobs that are pro rated based on their fte.

So it x job paid 400k and they are 0.5. They get 200j to work 20 hrs a week with pro rated vacation

I’m hesitant to post here since everybody seems to make 99 percentile.

Not terrible. 32 hours a week for 44 weeks (assuming taking 8 weeks off come to just about 500k). Malpractice should be around 15-20k and can be written off as 1099 so it’s really only costing you 10.

It’s fine job as long as u get Fridays or Mondays off most of the time. Nothing wrong with it since u are paid hourly.
Agree with others. At the end of the day, they can only screw you so hard with a firm hourly rate, and $350 is pretty good. So where can this go south:

1) routinely running over 8 hours can be a grind, and because there's no OT rate after 8 hrs, there's little incentive to be done on time.
2) Could be that the cases are complex, variable, with bad surgeons, sick patients, and a pace that doesn't allow you to eat/drink/pee/return a call.

You won't really know about these things until you start.

Seems just fine, assuming that it's a place you want to live/work, and the day-to-day practice (acuity, environment, surgeons, facility) is acceptable.

I'd like to see malpractice covered.

+/- on health. A lot of 1099'ers in a mommy track job have a working spouse that provides health benefits.

And, just throwing out there, if this is a scheduled job with regular hours it's probably skirting the edge of legality for it to be 1099. This is a pretty common arrangement but strictly speaking it almost certainly should be W2 if the law is followed.
 
Can we get rid of the mommy-track nomenclature? As a daddy-track employee I take offense to it. But only a little since I don’t miss a single one of my kids commitments.
U can be like me work 100 hrs and still pickup kids , go to their awards ceremony, baseball games, tennis, cheer. I don’t recall any missed events. I even swing deals for someone to cover me for 3 hrs to go to their events. And I pay them back.

U make time. I literally got off the plane last night from 4.5 day quick Italy trip, worked for 6 hrs this morning (I was first out) and ran both kids carpools in the afternoon. That’s how u make time. The sahm moms are all drunk at the country club early and thanked me. Early Friday drinking in the neighborhood.

I have a colleague who complains about time. Says they will never get time back as the old saying goes.

Well u can always make time. Guy got his 6 year old twins bday party in Miami at 12pm. Got him 11k cash to do Sunday 24 hr call beeper. All guaranteed. Straight cash homie. Got him another $5500 beeper to take call Friday night (tonight) for 10 hrs. He didn’t want to do it. He could have worked (highly unlikely to work more than 1-2 hrs tonight). Driven to Miami at 10am for kids bday with baby mamma #2. Driven back up early Sunday to take call 2 hrs away. He’s got EV that can self drive him also even with his sleep apnea. He’s worried about falling asleep at the wheel. Sometimes u get lucky and put sunglasses on. The in cabin camera doesn’t detect if you are asleep with sunglasses on . It detects head movement more. This dude needs to make 850k just to break even with his 5 kids and 2 baby mammas and 3 homes one in north Florida one in south Florida and another home in Georgia. He needs the money.

Full time w2 Military doc who has his own mommy track govt anesthesia job (govt pays when he’s deployed or has training weekends) who does side gigs took those two calls the other doc didn’t want. He’s got 3 kids of his own. Someone always takes the cash. I seriously don’t know why this place is throwing cash left and right but we will ride the gravy train!

So u can do it all when u are full time or daddy doc and not miss a beat. There are many ways to the end game.

Now u may be dead sooner with one method over the other method. That’s another subject.

I'm doing much better than that being by myself. Laughable offers from these groups.

Is this a 7 on 7 off gig? No way they are making 400 working 40 hours during those weeks on.
They work 6am to anywhere from 2-430 at the latest but usually around 3- they’re the last to go before we just get down to call team. I’d bet they work 45 hours a week on average. They do tend to get the less desirable assignments like GI more than the rest of us - but the two drs we have now as day docs don’t do blocks so we can’t always use them elsewhere. Some of the guys in my group loathe GI so I bet that’s why they cut them a nice deal 🤷‍♀️ idk happened before I got there. One of them might retire at the end of next year so we might need another then.

570k 1099 especially with healthcare taken care of. Is really worth 100k more than a w2 job (if married)

I know that sounds funny to some people. But we all know the tax games as 1099.

So ur 570k is the same as a 670k w2 job.

This is like my "avoid migraine" track with 1 week a month locums in Texas. I'm single, but I stopped taking call, and that got rid of one cause of migraines. I tell 2 groups which days I want to work or not work, and I can take whatever days I want to off to get stuff done. I work until 430ish with one job, I can work a half day until 12ish (often earlier) or a full day (until about the latest 3) with another.
